DFA E-PASSPORT APPOINTMENT SYSTEM BY INTERNET

(THIS SERVICE IS VALID ONLY IN DFA MANILA)
Only applicants for passport renewal may apply for the DFA ePassport Appointment System.
(New passport applicants and others not qualified can apply for the Machine Readable Passport Online Appointment System in the DFA website)

PROCEDURE

1. Qualified passport applicants may apply at the DFA ePassport Appointment System by visiting the website: www.dfa.gov.ph, click on the ePassport appointment icon and fill out the online request form and submit;
2. ePassport applicants applying for appointment must provide the following information in the online request form:

a. Full name of applicant (first name, middle name and surname)
b. Date of birth
c. Place of birth
d. Latest Passport Number and Date and Place of Issuance
e. Valid personal e-mail address

3. The Applicant must wait for the confirmation of the proposed appointment within 5 days from date of request, and take note of the date, time and reference number that will be provided by DFA-Passport Office before proceeding to the DFA for the filing of the passport application. Unless the applicant confirms his or her schedule by replying to the email, the appointment process will not continue.
4. The Applicant must be at the DFA Passport Office (Window 28 at the DFA Processing Area for those with appointments) at the confirmed designated date and time for the filing of the passport application.

IMPORTANT:
The Department of Foreign Affairs (DFA) wishes to remind the general public that applications for the new ePassport will be accepted starting 26 August 2009 on a first-come, first-served basis.

The following guidelines apply:


1. Only 100 ePassport applicants can be accommodated daily.
2. Only applications for renewal of passports will be accepted until further notice. In the meantime, first-time applicants may apply for Machine-Readable Passports (MRPs).

3. Interested applicants should set an on-line appointment through the DFA website http://www.dfa.gov.ph. Applicants are requested to fill up the required fields on the online appointment page at the DFA website; otherwise, the online appointment process will not proceed.
After internal verification, the DFA will inform prospective applicants of their schedule by electronic mail (email). Applicants will be asked to confirm their availability on the specified date.
4. e-Passport requirements:
• Personal appearance
All applications during this period shall be filed by the applicant personally to the exclusion of third parties.
• Old passport with one (1) photocopy of old passport data page
• A valid identification card
• Duly accomplished passport application form

Applicants may use the current MRP application form for this purpose, which is also available in the DFA website
• Passport fee of P950
